Intake
Ryan chose a psychiatric intake with a Nurse Practitioner, typically lasting between 30-55 minutes. At the end of his appointment, he was given a prescription for laboratory studies, therapy ideas, resources to learn from, ideas on helpful supplements, as well as prescriptions for both laboratory studies and medication for his conditions. All adult patients must do a virtual psychiatric intake with a psychiatric NP (Nurse Practitioner) or MD Psychiatrist (the patient chooses which level of practitioner they desire, but costs vary).
Most adults will do a full virtual intake, which lasts anywhere from 35 to 55 minutes.

Ryan chose to do his intake with a psychiatric NP and it lasted 43 minutes; at the end of the appointment Ryan received a prescription for labs as well as two medications, with specific instructions on how to start the medications and side effects to watch for

Side Effect Issue
Three weeks after the initial appointment, Lisa and her mom texted the office on Monday at 10am and complained of a newly developed rash, likely from a newly prescribed medication. The psychiatric NP, LIsa, and her Mom all spoke by phone later that day and decided on a new plan: to stop the recently started medication, let the rash resolve, and try a new medication. This exchange, including text messaging back & forth and phone call later in the day, lasted 17 minutes in total.

Follow Up Appointment
At the end of the initial appointment, a virtual 25 minute followup appointment had been scheduled to go over updated symptoms and medication side effects, but after the rash occurred, this appointment was then changed to 5 weeks out from when Lisa started the 2nd antidepressant.
Regularly scheduled 25 minute follow-up appointments are sometimes preferred by patients over the piecemeal, “bill by the 5 minute increment” approach to care. This is especially true in patients in the early stages of new treatments or patients with more complex psychiatric symptoms

25 minute followup: $175

Story
Admin Time
At month 4, Lisa’s Mom reached out to the admin team and requested that a note be written to Lisa’s School recommending an IEP plan be put into place. The admin team was able to check with the NP as well as review the patient’s notes to formulate a letter on our stationery recommending a EIP that included Lisa's diagnoses and treatment plan. Story
Follow Up Appointment
1 month after beginning his medications, Ryan chose to have a 25 minute followup with his psychiatric NP to discuss updates and potential changes.

25 minute followup: $175

Story
Admin Time
At month 8, Ryan’s insurance stopped covering one of his medications, which he really felt was helpful. He asked us to perform a prior authorization with the help of our admin team, who spent considerable time filling out specialized insurance forms to get the insurance to agree to pay for the medication after all.
Admin Time billed at $5 per 5 minute increment; x45 mins = $45
Story
Text Based Clinical Care
At month 11, Ryan reached out to his NP about increasing his medication for insomnia. They texted back and forth, which took 12 mins of clinical time:
12 mins billed in 5 minute increments with the psychiatric NP

$105

Story
Ryan’s total yearly cost for his treatment was:
This assumes only 10 months of $49 monthly subscription fee (2 months subscription fees were waived for Ryan due to an intake and/or a 25 minute or longer f/u occurring during those months. This only applies to the month the appointment occurs in and multiple appointments in one month do not give credit in subsequent months)

Total: $1, 353

Story
Special Notes on Pricing on this Case Study:
1
These single patient intakes (up to 55 mins) are for adults without any accompanying family members; adults who want a friend or family member with them, or children/teens (who must also have some parent involvement) last 45 to 90 mins and are charged at higher rates.
2
Virtual f/u appointments of 25 minutes are required with all patients at least once per year (so if none occur the first 364 days after the 1st appointment, we would need another on or before day 365).
3
Intakes with a MD instead of a NP are also an option, but rates are higher.
4
All prices are subject to a "inflation adjustment" or "price indexing” on February 1st of each year. This means that all of the prices listed on the site are subject to a price increase identical to the previous year’s inflation rate according to the Bureau of Labor Statistics (BLS) Consumer Price Index (CPI).

Story
Intake
Lisa and her mom did an initial virtual intake with a nurse practitioner, which lasted 75 minutes (intakes with a patient and family member usually take anywhere from 45 to 90 minutes). At the end of their appointment, they were given a prescription for laboratory studies, therapy ideas, resources to learn from, ideas on helpful supplements, as well as prescriptions for both laboratory studies and a medication for Lisa’s conditions.

Intake with NP: $495

Story
Collateral with other providers
The following day, the NP was able to connect and share insights over an 11 minute call with Lisa’s therapist who has known her for many years.
NP Clinical time is billed in 5 minute increments at $35 per 5 minute increment, and the NP put in 13 minutes of clinical time (time on phone in addition to 2 minutes rereading the note to prepare for the call):

$35 x 3 five minute increments = $105

Story
Labs
Two weeks after the intake, Lisa’s intake labs were complete. The labs were analyzed by the psychiatric NP and it was necessary to convey to Lisa that her Vitamin D3 level was low, and she and her family were given further treatment recommendations via email (the family’s preferred means of communication).
Time to analyze labs, email the family, and send in a new Vitamin D3 prescription: 9 minutes, billed at 5 minute increments:

$35 x 2 five minute increments = $70

Story
Side Effect Issue
Three weeks after the initial appointment, Lisa and her mom texted the office on Monday at 10am and complained of a newly developed rash, likely from a newly prescribed medication. The psychiatric NP, LIsa, and her Mom all spoke by phone later that day and decided on a new plan: to stop the recently started medication, let the rash resolve, and try a new medication. This exchange, including text messaging back & forth and phone call later in the day, lasted 17 minutes in total.
NP Clinical time is billed in 5 minute increments at $35 per 5 minute increment: 17 minutes rounds to 20 minutes:

$35 x 4 five minute increments = $140

Story

This took 33 minutes of admin time.
Admin Time billed at $5 per 5 minute increment; x35 mins:

$5 x 7 five minute increments = $35

Story
School Collaboration
Later that month the school requested a meeting with the psychiatric NP to discuss more ideas to help Lisa; this was approved by Lisa’s mother ahead of time, and occurred via a 25 minute scheduled virtual meeting.
25 minute virtual between the psychiatric NP and Lisa’s school:

$175

Story
Maintenance Treatment
For the next several months, Lisa did well and only ongoing monthly refills were needed. For these, Lisa’s mom reached out by text to request a refill of Lisa’s medications.

No additional cost beyond the monthly subscription fee!
